When filling out this form and you have multiple items to donate, feel free to include all items in one form submission. However, you are more than welcome to submit separate donation forms for each item.
When mailing your donation, please include any brochures, pictures, signage, etc. to help in marketing your donation at our event! For wine, don’t forget to include tasting notes, production information, and/or any promotional or display items.
Also, if you are local, we would be more than happy to coordinate a mutually agreeable pick-up time and date!
GIFT CERTIFICATE DONATIONS
- Please make gift certificate valid as of 9/23/2021, and good for at least (1) one year.
OTHER DONATIONS
- Once you have filled out the form below, please print your email receipt and include it with the item(s) you are shipping.
ALCOHOL DONATIONS
- Industry alcohol donations may be shipped via UPS. Please clearly mark the shipping with “Cork & Barrel.”
- We are prepared to receive your alcohol donation immediately following receipt of your completed donation form. Alcohol will be stored properly in advance of the event date.
- Please print your email receipt and include it with the item(s) you are shipping.
- If you are local, please deliver to the address above, or call 541-306-4669 to arrange pick-up.
After completing this form, we will automatically send you a confirmation receipt and will follow up with a formal Thank You letter via mail.
